Connolly Primary School welcomes enrolment of students in our community.
Connolly Primary School is a Local Intake School. Any Out of Area applications are subject to availability, will be considered on an individual basis, and must be approved by the Principal.
If you would like to apply for enrolment at Connolly Primary School, please complete the Application Form and return it to connolly.ps@education.wa.edu.au along with copies of:
- Birth certificate;
- Identity documents; (Including passport and visa documents if applicable)
- Proof of address; (a rates notice, lease agreement or utility bill).
- Immunisation records;
- Court orders if applicable;
- Details of any disability;
- Any ongoing medical condition and procedure to be followed during the school day.
Please ensure all documents are sent in PDF format, not jpg’s.
Your application will be subject to approval by the School Principal.
